Creatinine is of clinical importance of renal clearance. A high serum creatinine rate (associated to a high urea rate) corresponds to a decrease in renal glomerular filtration (GFR). The determination of the creatinine clearance is a valuable parameter as a measure of the glomerular filtration of the kidney.

A GFR of 60 or more typically suggests normal kidney function, while a GFR of less than 60 usually indicates kidney disease progression. A GFR of less than 15 could indicate kidney failure, which will likely require dialysis or a kidney transplant."

Enzymatic methods for creatinine determination have been developed in an attempt to overcome some of the problems inherent in the Jaffe-based methods. The enzymatic method was found to have certain advantages over Jaffe’s kinetic method, and especially lack of interference with substances such as glucose and bilirubin. An enzymatic of creatinine analysis were comparable with respect to performance in the presence and absence of interfering substances glucose, bilirubin and imprecision. Excellent on-board stability and calibration stability.
